I have a water cooled x1900 xtx. Do I need to put heatsinks on the bga ram or will they be ok on their own?

Use the original RAM sinks. I'm pretty sure they're separate from the main HSF just like the HSF.
Clean up the original thermal pads and use any thermal paste of your choice.

The Samsung GDDR3 1.1ns on X1900XTX does need cooling (in form of any piece of metal on top) even at stock speed.

Could be that he bought one of the Sapphire stock watercooled cards that if I remember correctly don't come with ramsinks at all :)
