Is it possible to point programmes such as clearview/acrobat etc running on
a pocket pc to documents stored on a CF/SD card? or point/move the my
documents file to a CF/SD card so I can store more and larger documents and
not clog up the file store!

Yep... simply create a folder on the card named "My Documents" and move
or copy the files there.
Note that files cannot be "synced" to a card, but they can be copied
using a card reader on the pc (recommended) or using the pc's file
explorer while connected (not recommended)
In addition, Acrobat reader attempts to "tag" most pdf files during the
sync process and does so only during the sync process. Because of that,
you may have to transfer them to RAM via the sync process, then move
them to the storage card after the sync using pfile explorer.

didn't answer your base question... files in the card's "My Documents"
folder will automatically disply in the app's file menu... their icons
will contain a tiny "card" outline to indicate they are not in RAM
