Hi there,
I am trying to set up my C: partition so that it holds only the O/S and
programs, no data.
How can I set up the Documents and Settings including My Documents folder
in the D: drive....so that when I save all my files and documents that
automatically are pointed to the My Documents folder, how can I get that
over to the D: drive?

Two methods, First and perhaps easiest is download & run an XP
powertoy, Tweak-UI. It has a category My Computer, Special
Folders where you can re-direct standard profile components like
My Documents, Pictures.
-or-
You can simply drag and drop the folders from within Explorer to
their new destination (Drive/Folder) and choose the Move Here
context menu option to permanently re-direct them.
Tweak-UI for Windows XP (SP1+), here:

I would only move the Personal data folders and not worry about
Documents and Settings as a whole. Many programs & functions
"Expect" the D&S/{Username] to exist and will recreate it, if moved.

This KB article tell you how to put the Documents and Settings Folder on a
different drive when installing XP

as wiliam said, this KB artical will explain how to move it manually, but
read the fine print! it is basically an unsupported method, and not for
the novice. but it does work...
I'm not too sure it really buys you too much tho; if you do lose the
system partition and need to do a repair install it'll change the damn
thing back to the C:\ drive again.
