insert data from html form to txt or mysql

I'm interested if it is possible to do following:

1.Form (.html with javascript (because of child menu like here
www.kupiauto.com))
2.Insert data from "form" to a .txt or some format so that i can easily
insert bulk(complete list) in mySQL on web via desktop PC.
