Windows - No Disk, when removing media from cd drive

Exception Processing Message 0xc0000013 Parameters 0x75CE023C

0x84C40C84 0x75CE023C

With three options: Cancel Try Again and Continue
I need to click Cancel or Continue twice for the message to disappear (I have not tried Try Again).
anyone else with this this problem?
