Ok dude, goto control panel.

Then Security or whatever its called, not near my vista atm.

Then get into Windows Defender.

Somewhere there should be a button of tools or something suchlike.

There should be a button to take you to currently running programs and a seperate one for startup programs. Choose the startup programs and look for unknown programs.

If no suspicious startup look at currently running

Ok had a look, didnt see anything suspicious or unknown :(

I also disabled 8 items in MSCONFIG (Startup Progs) and no difference there either.

Could start disabling hardware but not sure wat i should disable or which are safe to do so

Thinking maybe format and fresh vista install, but not sure how lol

If you can run in safe mode (all drivers disabled) then it sounds like you a driver issue. Remove all your install drivers and try re-installing your display, sound, and chipset drivers after booting into normal windows.
