I change the settings in one of the game profiles in the Nvidia panel and hit apply. At some point i will switch to the Global settings for general work. When i'm going to play that game again, do i need to choose the profile, or does it automatically apply the settings once the .exe is run?

If you go into custom rules you can set a game profile and have it load up automatically when you start a game

all game profiles auto load when the game in question loads.(yes it detects the exe start...i tested it and it works.)
