windows does start. freezes when starting up. vista

ok i have vista. and when i start up my pc freezes when the green bars go across the screen. and then when i start up again its says windos cant start software or hardware problem. please insert disc and repair. and i did that 2 weeks ago but it has happened again so what do i do? and safe mode works
