hello there
i also have a maximus mobo
unfortunately i had the same problem 
after some searching, i removed the plate of the card and saw that the biggest chip on the card was burned heavily...
so i bought a Xfi extreme gamer 
i would have returned it for warranty, but then i had to send the whole mobo with it!
I'm hoping you have another problem, but it wouldn't be bad to check it out
