Pop the side of the tower off, and try to determine where the noise is coming from.

As far as i can make out it is coming from behind the usb ports ( WEAR ALL THE CABLES GO IN ) ps i wear hearing aids

Is there a case fan right there? Or does it sound like it's coming from the PSU fan (likely right above those ports).

If you've got speakers plugged in, try unplugging those. It may be interference.

I took the speaker lead out altogether and it is still the same

Is there a fan on the tower in that area? If yes, blow the dust out of it, then apply a small amount of silicon oil (or something similar) in the fan.

It sounds like the fan is going bad. If it gets too bad, it can be replaced. Case fans cost ~$10.
