Western Digital (WD) Caviar Model Suffix?

My current WD Caviar is dead. I went to install WIndows XP on it an it will not work, after trying for 11 hours. I used a diagnostic tool to check for bad sectors, and there were thousands in every segment of my harddrive.

I am doing the WD Advanced RMA program where they ship you the harddrive and you send the defected one back. But, it is asking me for the model suffix. It gives me a list of 6 options to choose as the suffix and I have no idea where on the box these series of letters/numbers is located. It is neither the serial or the model number (atleast I don't think so) Has anyone had experience with this and knows where the suffix number is?

According to this document http://www.wdc.com/en/library/2579-001028.pdf it's the "characters to the right of the dash following the model number".
