How to ID which partition is on which drive

ok, this may sound stupid, but i havent been able to find the answer through google or forum search.

i have 3 hard drives on my system and i've partitioned two of them into two drives. so now i have a total of 5 partitions, but its been so long since i've done it, i cant remember which letter/drive partition is on which physical hard drive. i'm going to be upgrading hdds soon and will more than likely be replacing the smallest hard drive. the problem is that i partition the drives in similar sizes and cant remember which partition belongs to which drive. i hope that wasnt confusing and that there is a simple solution. please let me know
