How to figure out which drive is connected to which port on a 3ware 9650SE?

We have a 3ware 9650SE-8LPML card installed in a SuperMicro 1025W-UR chassis, with 8 hard drives installed. Yesterday the card reported a dead drive on c0/p4. But when I went to replace the drive I found the dead drive was not located in the 5th drive bay. This chassis has two rows of drive bays, 3 on top and 5 on bottom, and the dead one was in the middle bay of the bottom row -- not the 5th no matter what order I count.

So my question is how do I know which drive bay is connected to which port on the 3ware? I've tried set identity=on but it didn't work. Any suggestions?
