Primary hard disk smart status bad

I have a pc Tiny
Running windows XP and a lousy 30 gb Hard-Drive
I got the blue STOP 0000007 or whatever it was, but then I formatted my D Drive and this problem disappeared, the main thing was.. each time I tried copying stuff to the D Drive it would kill the PC and I would get that STOP error message..
Now it seems each time I log on I get the.. Primary Hard Disk - S.M.A.R.T Status BAD Back-Up and Replace..
And sometimes I will be on here just doing normal stuff and it will all of a sudden die, and I get the STOP Error Message.. but this time around saying.. Kernel Stack- In-page Error
If anyone could give me any tips, or help, it would be greatly appreciated :)

I really, really do not want to sound rude, but this is a "Duh!" moment.

I'd do what the error message said: "Back-Up and Replace.", and I would do it now.
