Flash Drive is an Unknown Device

Hello, I have a huge problem with my life-carrying-flash drive. It seems that after about a few months of ownership I used it on a WIndows Vista OS and then it can't be recognized on any other OS including the same Vista OS that started the problem. It shows up in Windows Device manager as an Unknown Device, so I know there's nothing wrong with the actual hardware. Any advice on what I should do to make it work again?

    I've ran into a problem like that, and could not fix it untill i formatted it, with a USB pen drive tool.

    The pen drive may be bad and need replacement.
