I built an MCE2005 HTPC using a GA-EP35-DS3R and an ATI 4850 and am using the ATI card's HDMI output into my TV. I can not get the MB's headphone port to work at the same time; is this possible?

No since the ati 4850 audio interface and the motherboard audio interface are two different interfaces. Windows has to select 1 of the 2 as the audio output. you will be able to switch between the two but not use both at the same time.

If the OP is looking to use a headset for gaming at the same time as normal sound is going to the TV then I would suggest a USB headset.
