Pen drive not recognized

When i put my kingston 4gb pen drive in to any system it display
"usb device not recognized" and
"one of the usb devices attached to this computer has malfunctioned"

there's several possibilities
1. the drive hasn't been formatted properly and you need to do so yourself
2. the drive went kaput
3. the drive requires special drivers to work on any pc (very unlikely)
