Flash disk was unable to complete the format

I suprised for this problem saying "Disk is not formatted, please formatt now", when i try to formatt the disk, in both sides quick and full formatted, a messages suddently puped as shown, disk is unable to complete the formatt.

Please is there any body who knows how to solve this problem, help me inorder to help you.

"help me in order to help you" - what's that supposed to mean?

Is this a new flash drive or used?

if that one doesn't work, you might have a hardware problem with your flash drive
