That is a PCI card that is an IDE drive controller supporting the Ultra 66 standard, providing two IDE ports, with each capable of handling two IDE HDD's on an 80-conductor ribbon cable for data. Here are a couple of info sites:

This is an older card and machine, as you know already. I could not tell from a quick look whether it includes "48-bit LBA Support" needed to handle HDD's over 137 GB. Nor can I tell whether it can be used for a boot disk, or only for data disks.
