Is a cm hyper 212+ enough for i5 2500k @ 4.5GHz

Assuming you have decent conditions (un-crazy ambients and regular cleaning) then yes, that heatsink will serve you very well. Sandy Bridge is not a huge heat producer at 4.5ghz, and the 212+ is very competent. You should be able to daily OC at that speed unless you are very unlucky with your chip.
 
1- What case?
2- What budget?

Hyper 212+ is a good cooler for the budget, I did 4.6GHz for bench in my CPU but if you can afford something more better I'd go for the Titan Fenrir, Venomus X, Archon.

The swiftech Polaris 120 is getting very positive reviews, as well the Thermaltake Frio is an astounding air cooler to consider.
