Start Disk Management:

- Start -> Righ-click "Computer" -> Select "Manage"
- Click "Disk Management" in the left pane

Scroll down the the bottom of the middle pane. If the "C:" and "D:" drives appear inside the same rectangle (labeled "Disk 0", for example) then they're on the same drive. If they appear in separate rectangles (labeled "Disk 0" and "Disk 1", for example) then they're on different drives.
