ok. here is my system.

EAH 4850 512mb pci-e video mem
3.4 GHz Pentium 4 HT
1.5 GB RAM
550 W PSU

I am trying to play fallout 3 on the ultra- highest settings on around 1280 x 1024 resolution, but it keeps auto detecting the game at medium settings... My first upgrade was to the 9600 GT 1 GB and it detected it at High settings. That initial makes me think its Video Mem, but I thought you only need the 1gb if you are running at like serious resolutions... please help me!

First off Google. Second Auto Config settings for games seldom work, seeing as your 9600GT prolly couldnt run it on all high settings. If it keeps changing them back once you've set them then you have a problem. If not. meh. Its not the amount of video memory, 512mb is more than sufficient for that resolution. I run at 1920x1080 with a 512mb 4850 and that precludes me from using the highest texture settings or AA in Far Cry. BTW your CPU is a huge bottleneck with that setup. I went from a Pentium D 820 OC'd to 3.9GHz to a Core 2 Duo E7400 at only 2.8GHz and saw my FPS in Far Cry 2 jump from 20-25 to over 50. Since overclocking to 4.05GHz I almost never see it drop below 60.

1. Fallout 3 will auto set to high settings with any Nvidia 8600GT + card, its a deal with Nvidia.

2. the reason it sets to medium with the 4850 is due to your CPU not GPU. Go into options and set the game settings to what ever you like.

Wow thats naughty, Can a 8600GT even play properly with all on high ?

verbatim from the manual

"On a geforce 8600 or higher you will be able to turn on all the Nvidia special features at higher resolutions of the game"

I popped my old 8600GT (before i sold it) into my PC to test it out and it defaulted to high settings. The game was playable. Fallout 3 on just high settings is not really a graphic hog. Too bad not everyone can optimize their games as well as bethesda.

*edit; its also Fallout 3 weekend on STEAM. Its 50% off. Celebrate the 4th of July by killing everything that walks, talks, and crawls in our nation's capital! :D
