Disk read errors are usually associated with corrupt files or a HDD that is dieing. I would suggest copying your data off and then trying a clean install - if the problem persists then its more than likely your HDD failing.

Three possibilities. You can pull the drive out and connect it to another computer. Or you can install another drive, put an OS on it, make it bootable, and use it to backup the data from the original disk.

Or you can make a BartPE disk. (Google it.)

You could try running Scandisk from either a USB Dos or a bootable floppy,,you might get lucky,or as mentioned buy a new hdd and copy stuff to it after installation..
For future consideration,,get a hold of Norton Ghost and Ghost your primary partition regularly then when you have snafus you will have a ghost that can restore your system, primary partition in a few minutes... :)

Try changing the cable for your hard drive.
