Have you set it properly in the BIOS.

If it is slowly loosing time, you should replace the CMOS battery on the motherboard. It is usually a very thin coin size battery on the motherboard.

What mobo do you have?, like dpaul8 said you need change this on your mobo. Look for the manual of your mobo, in this you can find the correct battery model that you need buy to change your current battery.

Clock stops during clearing of CMOS and so it will be a few minutes late than the actual time after the process.

You can set the time to the correct value in the BIOS manually.
