"HWMonitor – provides volts and temps of your hardware
CPU-z – provides real time processor frequency, vCore volts, Ram speeds, and temps.
Core Temp/Real Temp/Speed fan – provides temperatures of your hardware.
SuperPi/HyperPi/Prime95 – provides the ability to produce a full load on your system to monitor temps and clock speeds in the previously stated programs.

1.30-1.35v should push it to 4.0-4.2Ghz but that depends on chip. Highest I got without increasing voltage was 3.8 only

your motherboard should have some monitoring software

It depends on your motherboard's feature in the BIOS settings. Mine is also i5 2500K OC'd to 4.7 GHZ vCore 1.375V using Biostar TZ68A+RCH, CM hyper 212 plus cooler.
