I am thinking that it might be the graphics card, which is unfortunately no covered under warranty at the time. It is an EVGA Nvidia 8800 GTS pre-overclocked. It is not an overheating problem, temps are within normal range for the CPU and GPU. As you can see I play on a fairly big monitor and I'm thinking it might have just been taking its toll on the card. The card is close to 1 1/2 years old.

Ugh, that is bad. The first thing you can try is unoverclocking it. I had a similar thing with my 6800GTs, at first they could do 400+, then 400, then... So, it is a good first thing to try. You could also try uninstalling all video drivers then reinstalling, just in case a driver is corrupted.
