T2-FM-Just Friends-- Anal level? (possible spoiler)

I played this once without the walkthrough and could not get enough
loot. Playing again and following the walkthrough and can't get the New
Objective of taking the journal to the Dr's Wife. Any ideas why not? Is
she waking up when I go down the ladder or something? First time through
I BJ'd her and figured that was why.
Also, the first 2 objectives do not check off. Where is the note
outside the cave?
