Urgent: Disk read error has occurred, press ctrl alt delete to restart

"Disk read error has occurred, press ctrl alt delete to restart" at bootup

So, i try to connect the hard disk from master to slave ... then it works for 2 - 3 days

Again it goes dumb .. now not at all it works in any mode

How to rectify this problem
