My PSU: CORSAIR CMPSU-750TX 750W ATX12V / EPS12V SLI Ready CrossFire Ready 80 PLUS Certified Active PFC Compatible with Core i7 Power Supply

My GPU: XFX HD-577A-ZNFC Radeon HD 5770 (Juniper XT) 1GB 128-bit GDDR5 PCI Express 2.0 x16 HDCP Ready CrossFireX Support Video Card

I am going through some tests to make sure everything is installed correctly. Can someone tell me the proper way to plug in the card to this psu?

Right now, I use one cable with a 6 slot head to connect to the card. However, in the card box there is a piece with two 3 slot heads (4 slots but only 3 have pins, 1 is empty) that merge into one 6 slot head piece.

This adapter thing has a 6 slot head that looks identical to the cable I have plugged in right now.

Is there a difference between these two types of connections? Should I use one over the other?

I found the name of the cable. It's a 4 pin to 6 pin adapter. Looks like this:

Should the two 4 pin pieces be plugged on the same cable or two seperate?

In response to your suggestions, I have disabled Aero and it still crashes. How do I uninstall the KB952287 update in windows 7?
