What's wrong with my card?

I have a GTX 285 at stock with HR-03 cooling. When i play a game, my screen would freeze all of a sudden have low fps. When i restart the game, it'll still be low fps. Not untill i restart my pc does the problem fix it self. I checked with evga percision and my core clock is apprently stuck at 400 mhz after the crash.

Help? I tried clean driver install and it didn't work. I know temps arunt the problem.
