If the fan is mounted parallel with the motherboard, it has to blow air downwards, onto the motherboard.

If its mounted perpendicular to the mobo (like on a tower shaped cooler), most people mount it in such a way so it blows air into the heatsink. It's seldom used to suck air through the heatsink.
