Dual monitor low profile graphics card?

Hello,
I currently have an hp slimline desktop, model is s3431.uk. It has a Radeon HD3450 graphics card with single dvi port. There is no other free ports from what i can tell in the rear so i need to replace the grapics card with a new one which has 2 ports, I need either 2 vga or 1 vga & 1 hdmi ports. But which graphics cards will be able to fit into the same slot and also be low profile? The motherboard is

As I want to use two monitors.
