Does my graphic card die? 512MB GeForce 9500GS (solved)

My computer is about 2 years old, and today when I ran StarCraft 2 it just play slideshow to me.
even when I lower to the lowest setting it still laggy to a point that I cant play properly.

Genuine Windows Vista Home Premium with Service Pack 1 (32-bit)
Intel(R) Core(TM) 2 Quad processor Q9300 2.5GHz
3GB DDR2-800MHz dual channel SDRAM (2x1024,2x512)
512MB NVIDIA GeForce 9500GS
640GB 7200 rpm SATA 3Gb/s, two drives (2x320GB)

My graphic card has been replay by Nvidia for recall, so it's about 1 year old.
I have try to restart my computer and close all the program include anti-virus.

I am not sure if my card die, it was fine yesterday.
and i can still do other thing as watching video and open photoshop without problem.

Can anyone tell me what might be the problem?
If I need to replace my graphic card what will be a good choice around $100 USD

9500GS is not good enough for today standard, get a new card...

What is your PSU? brand? model?
Monitor? Resolution?

I am not sure what my PSU is, when I open the case on the label it write.
model PS-6301-02
DC out put 300W

My computer is HP a6560t
Monitor is Acer AL2216w 1680x1050

so if I use the card that Helltech(the previous reply) recomment
MSI and AUSU, i need to upgrade my PSU inorder to do so?

Yes you will, with your current PSU the 5670 will be the best card you can get.
