Rebate question

So I bought a Galaxy Gts 450 card and i wanted to send in the rebate. I was wondering which UPC code i am suppose to put the one that is on the left which has the name Nvidia Gefore GTS 450 GC 1GB DDR5 128 bit w/etc... but this one says warrenty void if removed or the one that has 12 numbers and a P/N number.

The UPC code is on the BOX not the card !! ( Mention this because of the Warranty Void if removed sticker is usually on the card to keep you from removing the casing which would void the warranty not sure why they would have something like that on the BOX the card comes in !!)
