Computer wont sleep, hibernate, ect. Have Tried Almost Everything!

So i've been trying to tackle a sleeping/hibernating issue with my computer.

The problem:
Whenever I press sleep it automatically wakes itself up.
Whenever I press hibernate the monitor and everything goes black then it reloads at my MOBO BIOS menu (restarts my computer).

I've ran through all the typical steps (device manager, power saving options, BIOS, ect.) and just re installed a fresh copy of Windows 7 onto my SSD.

I have ran powercfg -energy which gives me 9 errors USB related.

Being that I have completely reinstalled windows I don't think this is software related. I think something is wrong hard ware related.

Could maybe my hardware connections to my motherboard, ect. be causing this issue? It's driving me mad.

Can someone give me some more ideas to what could be causing the problem? I hate my computer being on constantly.

You could try singing it a luluby...or you could check the pin connections on your motherboard. They are very small and very easy to connect to the wrong pin.

I don't think it'd be caused by something being connected in the wrong place since the sleep has worked for about a year up until now. The main thing that I have changed in the computer recently was installing a NIC card onto the motherboard. This caused a little bit of disturbance when I accidentally resetting the CMOS battery.

The computer just did what it usually does.
In S4 (hibernation) the computer never completely turns off and comes back on at the main MOBO BIOS screen then resumes Windows
In S3 the monitor, keyboard, mouse go black and turn off but the computer itself never does. After a few seconds the MOBO LED's go solid and then the keyboard, mouse, and monitor resume.

The diagnostic was as followed:
