How do I setup 3 monitors plus one TV screen on eyefinity?

so use dp for one of your monitors and hdmi for the TV?

I have the exactly same video card & this set-up:

42" #1 - DVI-D to VGA adaptor ( but you dont need this since you have DVI Monitor )
42" #2 - Active DP to VGA adaptor ( this is cheaper than an active DP to DVI but quality is just as good at 1920x1080 )
42" #3 - DVI-S to HDMI adaptor ( this is from my old HD4890 but you do not need it since you have DVI monitor )
55" - HDMI ( Edit: the Sapphire Flex include an HDMI to DVI adaptor so you can still connect via DVI too ).
I still have an extra DP port & another Active DP to VGA converter but I can not make it stable. I can get 5 Monitors ( the 5th one is an LG 22" ) but this always blink no matter which DP port I connect it.

Let me know if you can make your setup work with 4 and if you can get a 5th monitor stable.
