Athion64 system

Hello,On the front of my Athlon64 system(In a Capricorn case) are two aux. usb ports. I have managed to damage the bottom port and I wish to replace the board.
Where can I source this board.
The board reads - MJZ -USB and ReverseRU94HB GW500.

It may be impossible to replace such a thing as they are usually custom made for the cases.
