Sapphire HD5750 Vapor-x Overheat Issue

Hello. I'm having serious temp problems with this card.
My idle temp around 55C-60C, at max load (crysis metro2033 furmark etc) 95-105C. Ambient temp are around 27-28. Of course these temps are not normal but i'm not certian if its because of my insufficient air flow in my case or the problem of the cards cooling.

The fan works normal, it stays %50 speed until 85C, above 85C goes to %100.
I also notice that the heatsink is very cold even when the card has 100C temp.

Here's a pic from my case. It's micro atx, it's cheap, it's have a bad air flow and the card has a tight place but can it cause such high temperature? (sorry for poor english) :

If the heatsink stays cool when you get high temp readings for the chip, then it probably lost proper contact. You should detach the sink and proceed as when installing a CPU cooler.

You obviously have issues with heat displacement. Just as pepe2907 stated above if the heat sink is cold to the touch even when temps run so high you need to re seat your heat sink with thermal compound and improve your overall case airflow. You may want to try a case mod below the GPU. Try putting an inflow air fan below the GPU. Also be sure to have proper exit airflow. You can always manually adjust your fan speed on your GPU. Also running your card continuously above 75c can cause damage. Make sure you get your temps and improve your airflow.
