Paired perfectly with headsets (Logitech, Jabra), laptop (HP), and car
(Prius). All worked well...
Suddenly - with no change that I can recall, the phone developed amnesia
and can't remember any of its friends. If I re-pair each time it works
as before, but only once.

Wasn't there a firmware update for the Treo 650 released recently?

Yes, it was issued to increase car kit compatibility. Prior to that
patch the 650 would not pair with the Prius, BMW, Lexus, etc. After, it
works fine. In my case (Prius), however, Alzheimer's set in post-patch.

Had the same problem with my new 2005 Prius and my Treo 650. Searched
the Palm One website and low and behold, a bluetooth car kit firware
patch is available (Sprint was not even aware of this issue or fix).
After I installed it (follow the easy directions), load it to your Treo
650, problems completely resolved. The phone works absolutely
flawlessly now in the car. Here is the site URL.....
