Graphic Card on a budget

Hi there, I was wondering if anyone had any suggestions for a low/mid level graphic card. My budget is about 250 USD for both a graphics card and a psu. My current psu is a 250w atm powering a small rig with specs as follows:
CPU: 3.2 Ghz Dual-core
GPU: 8400GS (mainly for movies and low on small games)
RAM: 4GB DDR2
HDD:1TB Internal
CDVD Drive

It's not really powering anything else aside from an external hdd and internal system specs. I'd like to know what psu/gpu would be the most affordable and best bang for your buck. I currently use this pc as a media center and have it hooked up to my HDTV. My res would be in the <1440 range and I would just like a stable framerate on high settings for some games (TESV, LoL, COD, BF3, Etc). I'm a modest gamer and that's all I'm looking for tbh - a modest framerate for the price. I hear the 5770 is good but I think my psu is a bottleneck for it...
