Need help and advice on upgrading Graphics Card

Hi im looking to upgrade my graphics card in order to be able to play skyrim and most of the upcoming games
my current specs are
Motherboard: ASUS M4N68T-M V2 - motherboard - micro ATX - Socket AM3 - GeForce 7025 - Socket AM3
CPU: AMD Phenom x4 955 Processor 3.2 Ghz
RAM: 4 Gigs
Graphics Card: ATI RADEON 5450 HD
HDD: 1 Terabyte
