The hd5870 is eyefinity capable. Who made it? XFX, Asus, Gigabyte? What and how many input connections does it have? All display port?
You should be able to run 3x1 + 1 monitor set up with that card. (Available monitor configurations here >> http://support.amd.com/us/kbarticles/Pages/gpu50-ati-eyefinity-display-configs.aspx
If your monitors do not support display port you will need to use 'ACTIVE' adapters. (list of approved dongles here >> http://support.amd.com/us/eyefinity/Pages/eyefinity-dongles.aspx )(passive adapters are only good for extending desktops, they do NOT work with eyefinity)
I'm running a pair of hd5870's and tbh, if money isn't an issue, I would wait until they release the new 7xxx series gpu's, early next year. http://www.tomshardware.com/news/AMD-Radeon-HD7990-HD7950-ati,14366.html and go for the hd7990

It only supports 3 monitors hence the need for an upgrade

And a TV with HDMI ports only

If I were to get an active display port to DVI would I be able to use a dvi to hdmi cable along with that?

do you have a crossfire capable mother? if so buy another 5870... if not then I think you need to upgrade your mobo... or buy another video card with support for six monitor there are several out there...

Your card should be able to run 3x1 + 1, it has 2xdvi, 1xhdmi, and 1xdp. Connect a monitor to each of the dvi connections, then connect the 3rd monitor to the display port of your gpu via an active dp/dvi adapter. These 3 monitors will make up your 3x1 desktop and can now be configured through CCC. Finally connect your tv to your gpu via hdmi connection. Make sure your tv is set to recieve a signal from your gpu. It should now appear in CCC for configuration of 3x1 + 1.

mobo is not an issue at all I have this

The only issue is these cards are no longer made and priced very high, higher so than a two 6850's. The main issue I have now is can I use an active DP to DVI cable then couple that with a DVI to HDMI cable and still get the setup I want. 3x1 + 1 extended

That's what I thought but this card can only have 3 monitors active at once and you have to use a DP to achieve this. After you have the 3x1 setup the fourth monitor will be detected but can NOT be activated. Also I only have a DP to HDMI cable and it is not an active one so I can't even do a 3x1 setup at this point.
