I am in dire need for a new graphics since my 220 GT isn't able to handle some games when there is alot of action happening so I am looking for the best bang for your buck graphics card.

Here are my specs:

Operating System: Windows XP Professional (5.1, Build 2600) Service Pack 3
System Manufacturer: MSI
System Model: MS-7309
BIOS: Default System BIOS
Processor: AMD Athlon(tm) 64 X2 Dual Core Processor 6000+, MMX, 3DNow (2 CPUs), ~3.1GHz
Memory: 3072MB RAM
Page File: 1176MB used, 3779MB available
DirectX Version: DirectX 9.0c (4.09.0000.0904)

Card name: NVIDIA GeForce GT 220

Currently, I am looking at the HD 4670 right now. But I am not sure whether or not the ATI Radeon HIS or the XFX ones will be compatible with my rig.

Other cards that I'm looking at is the Nvidia 8800 GTS, XFX nVIDIA GeForce 8500GT 512MB Graphics Card and a ATI Radeon 9800 Pro 128MB DDR.

And if anyone has any other recommendations for what card i should get then please post something :)

I am not a hardcore gamer, but I would like to play BF3,MW3 on playable,if not, medium or ultra settings.

I would also like to stream HDMI to my 73" or 70" TV. It has a resoltuon of 1920x1440 I believe. Not entirely sure about that part. But obviously my 220 GT can't handle that large of a resolution.

all those cards are a bit old...
I'd choose from ati/amd hd5000 or nVidia 400 series or even the hd6000/500 series.

definitely dont get the 9800pro, I owned that card like 7 years ago, it won't do you much good anymore...

None of those cards will do well with modern games at high resolution.
8800GTS would be the most powerful card that you are looking at if it is the 512MB version. Any HD4670 just as long they are PCIe will work as well.
8500GT and 9800pro are not upgrades from your GT220.

Well I could not find the sticker since it was placed on the covered side, and I did not want to go through the hassle of removing the PSU and everything else.

The Make and model is Ultra MH1225M12S. I believe it is a 350 watt on DC12V 0.26A.

I am just looking for a significantly better GPU than my 220 GT and one that would be compatible with my rig and if possible, include a HDMI port.

google doesnt find anything with that model number, except 1 forum post that says it is a fan made by Muhua.... 12V rating cant be right either... (edit actually it is right, for the fan lol)

anyhoo gt220 has 48 shaders, gt440 96 with higher clocks... 8800gts has 128 with clocks similar to gt220...

But then again your cpu isnt really fast anymore by modern standards and 350W psu cant support much more powerfull cards either, so idk...
