Patch panel pin/out from rj48x

Hello,
I am extending T1 circuits from a telco smart jack. I will be patching from the telco smart jack to rj48x jacks pins 1&2 and 4&5 via shielded cable using pairs W/bl bl/w and w/or or/w. Can you tell me what pins I would connect to on a patch panel where I will patch to the customer equipment?
