Newegg Rebate help?

I recently bought a psu, corsair cx430 http://www.newegg.com/Product/Product.aspx?Item=N82E168... and it comes with a rebate. Its my first time buying from newegg and ive never done a rebate. It says im supposed to send in the store receipt and original upc. Is the store receipt the one that comes with the psu and is called "packing list" or do i have to print it from newegg in order history? Also where can i find the original upc? I see two stickers in the psu box and im guessing its the big one with the name of the product. I also see a whole bunch of stickers on the box that the psu box came in. It has all these stickers from ups. Im guessing those have nothing to do with the rebate. Thanks
