Can a single gts 450 run 3 monitor surround on games with adapter

hey, i am wounders if a gts 450 can run surround view on games such as swtor, wow etc, i know since the video card only has 2 dvi ports i will proboly have to get an adapter

If so can i get at least medium settings running it

No. A card with two DVI ports only supports two displays, there is no splitter or adapter to make a single DVI port display to more than one screen. You need either an ATi card with 3+ DIGITAL outputs, or a second nVidia card.

In order to run three displays, two of them must be connected through Displayport.

Only one needs to be connected through DisplayPort, not 2.
