When you are saying about Custom i believe you are talking about non reference ones, the ones that comes with different cooling solutions and overclocked, but the gpu itself is the same, the different is only the cooling solution and the quality of some external components. you can take a reference gpu and change its cooler and overclock it.
 
Their are basically 2 designer/manufactures of most of the video cards used in the world today. There are a few others, but for our sake here, we have AMD (used to be ATI) and nVidia. They design and make the core processor. They then license outside vendors to build video cards using their graphics processor with a few guidelines they must adhere to for the reference design. After that, the vendor is pretty much open to build the rest of the card however they think is the best way to go, memory type and speed, clock speeds, PCB configuration and style or type, and cooling design. So basically, each different company who sells video cards is a "custom" maker. It's up to us, the end users, to research and pick the one that fits our particular needs.
