Display Driver Stopped Responding - ATI

All,
I am at my wits end. I have tried everything imaginable and I don't understand why this is happening.
Issue:
While playing a game or watching a video my video freezes, after a few seconds it comes back and I get notification that "Display Driver has Stopped Responding and Recovered". Occasionally one or more of my monitors will become artifacted. Rare occations system will BlueScreen

Adjusted TDR - http://support.microsoft.com/kb/2665946
Ran MemTest86 - No Errors
Ran StressTest on CPU, HDD, GPU - No Errors
Swapped GPU with Radeon x1900xtx
Swapped GPU with Radeon HD 5760
Removed Ram and Tried 1 at a time - No resolve
Swapped Ram for Mushkin 4GB (2 x 2GB) - No resolve

Any Logs or any other information required just Ask.

Sounds like cooling probs with GPU perhaps?

Try again with only one monitor running...

Try again with regular monitor setup and with side of case off and desk fan blowing into the case. Extreme I know but will help eliminate heating as problem.

I will post the last DMP when I get off work.

I have not tried running a regular monitor setup yet.. I will give that a try.
I don't think my temps are high at all, but it it still happens using a regular monitor setup I will try it.

Ok.. Little bit of an update.
First Off I did not find a .dmp

The link you provided explains how to set up to create a dump

Anyway, I got irritated one too many times and just re-formatted again.

Since I re-formatted anyway, I decided to test Win8 CP.

That was 2 days ago, and The drivers has not crashed at all.
I have triple boxed on EQ2, played BF3 in eyefinity. It has not stuttered even alittle.

So I don't know what the issue is / was, but it looks to me like it was completely software related.
