call up your service provider.

There isn't a default, there unique to the sim card AFAIK

Sure, it's 651187969 - but only if your IMEI # is 3511854/08/614036/5

PUK is the Pin Unlock Key for your specific SIM card, get if from your
Service Provider

He needs the PUK code, not the unlock codes. AFAIK, you can't generate PUK
codes.
