Video card issues

my computer screen looks like it is in \'Safe Mode\' this is not the case - can my video card be bad even though the fan still spins

No idea what you mean by "looks like it's in safe mode".

Do you have the driver installed? Have you set the correct resolution for your monitor?

Yes the driver is installed and the resolution is correct.
When a computer crashes and you go into safe mode and only particle of the screen is shown such as missing icon - this is what it looks like. I also have broken dash lines across the screen and the icons are poorly visible. I have to monitors and only one of them works. I switch the monitors plugged in the video card and now the other monitor works and not the other.
